Below are the texts of the short "hardship questionnaire" and the main questionnaire (which was 75 pages long in its
original printed form with space for answers).

It is presently estimated that the presentation of evidence in this case may conclude in January 1995, requiring availability to
serve as a juror through February of 1995.

Recess schedule
